TEAM ENTRY FEE:
Men's
Divisions $1000
Women's Division $700
Registration Deadlines:
Insuring your spot within a division will be based upon receipt of Team Registration
Form and Non-Refundable Deposit (1/2 total registration fee). Deposit is due on
August 25, 2008. Limited spots available.
Final payment along with Team Roster is due & payable no later than
September 15, 2008.

Registration & Check-In:
All teams are required to have a completed roster with players’ names and US Lacrosse #s
(if applicable) turned in by September 15, 2008. At check in, a waiver must be signed by
every individual team member. Please check in at least one hour prior to your first
scheduled game.
INSURANCE REQUIREMENTS:
There are three ways to be in compliance with the LAX VEGAS insurance requirements:

Provide proof of general liability certificate of insurance for the entire team, naming
Source One Events as additional insured

Provide proof of insurance through US Lacrosse (provide US Lacrosse # on Roster- MUST show card at check-in

Purchase LAX VEGAS tournament insurance from Source One Events through
Bollinger Insurance for a one time weekend fee of $10 per person at registration
